Grant Registration Materials for 2022-23
Welcome to the eco-friendly Grant Elementary School registration process!
Many documents are now online, as indicated below. Note that there are 7 documents that must be completed and returned to the Grant Office on or before Thursday, August 25, 2022.
Step 1. FOR YOUR INFORMATION: the documents in this section are for informational purposes.
- Membership Letter from PTA Co-Presidents, Principal and Membership Chair
- Grant Calendar 2022-2023
- District Calendar 2022-2023
- SMMUSD is requesting families submit the free and reduced meal application, if qualified, so the district may continue to receive funding for programs at our Title 1 schools along with some music programs.
- SMMUSD Annual Notice Booklet
- Annual Information for Students and Parents Packet
- Insurance Information and Enrollment Application (Use zip code 90404.)
Step 2. DOCUMENTS TO BE RETURNED: COMPLETE, SIGN, INITIAL as indicated in this section and email them to Lisa Allen at lallen@smmusd.org or you can print them out and return them to the main office. If you DO NOT have access to a computer or printer, please come into the main office and office staff will provide you with appropriate documents, but paperless is the way to go.
Please check off below that you have included these items by THURSDAY AUGUST 25th. There should be 8 documents returned in your packet along with the student emergency supply kits.
The following documents are available online. Please print, complete and return.
- Emergency Card Form (Grades PreK-5): Please complete 1 form per student.
- Student Emergency Information sheet, with emergency kit provided (baggy with an extra change of clothes, (non-perishable food items, and a comforting note and/or family photo.)
- SMMUSD Annual Notice Booklet Signature Page: Complete 1 per student
- Annual Information for Students and Parents Signature Page: Complete 1 per student
- Technology Acceptable Use Policy Signature Page: Complete 1 per student
- General Filming and Recording Student Release Form: Complete 1 per student
- PS Arts Photo Release Form: Please complete and return Complete 1 per student
- Free & Reduced Lunch Form: All attending students can be added to one form (originals must be turned in please)
