- Is your student currently enrolled in an SMMUSD school?
- Is your student's next grade 9-12?
- Does your student demonstrate the motivation, commitment, organizational skills, and academic skills necessary to work independently?
- Would your student benefit from a higher degree of flexibility and individualization, such as students with a special interest or aspiring Olympic athletes?
- Does your student face a particular challenge that makes classroom attendance difficult, such as a health issue, lack of strong connections, or a need to work (ie child actors)?

Note:
- Families should thoroughly review all information included in the Independent Study Application Packet for Prospective Parents prior to submitting.
- The Application Packet includes a program overview, description of the application process, eligibility criteria, FAQ’s, and Course Sequence information in addition to the application forms.
- Please complete one form per child you are interested in enrolling.
- Current Independent Study students who plan to continue in the ISP program need to complete and submit this enrollment form to continue for the 2026-27 school year.
- This is not the form for Independent Student Physical Education (ISPE). Please contact your middle or high school for information on this program. Please complete this application form ONLY if you are interested in enrolling your child in SMMUSD's Independent Study EDUCATION Program for the 2026-27 school year.

Written Agreements:
A key component of the enrollment process is a signed written agreement that must be obtained from the parent/guardian. This will be completed during an in-person meeting and includes the student, parent, counselor, ISP teacher, and school administrator.
