NEW ELEMENTARY STUDENT REGISTRATION
FOR SANTA MONICA AND MALIBU RESIDENTS

WELCOME! Registering your child in a school for the first time can be relatively easy if you are prepared. Following are a few guidelines to help you:

Registration takes place at your neighborhood school. Call the school directly for enrollment dates. (In order to find your school attendance area, call 310-450-8338, ext. 226.)
You will need to verify your address. All schools require a current utility bill (gas, water or electric -- not telephone). Other documents such as a driver's license or business mail may be requested to confirm your address.
You will need to verify your child's birth date with a birth certificate or passport.
You will need to bring complete immunization records for all students and a doctor's verification of a physical examination for Kindergarten and First Graders. Up-to-date immunizations must be completed before your may enter the classroom. (See immunization requirements following.)

Your school of residence will provide you with a packet of forms and information which must be completed and returned. Enrollment requirements may vary slightly at each school.

Immunization Requirements

K-12

To enter or transfer into pubic and private elementary and secondary schools (grades kindergarten through 12), children under age 18 years must have immunizations as outline below.

VACCINE
REQUIRED DOSES

Polio 4 doses at any age, but...3 doses meet requirement for ages 4-6 if at least one was given on or after the 4^th birthday; 3 doses meet requirement for ages 7-17 years if at least one was given on or after the 2^nd birthday

Diphtheria, Tetanus, and Pertussis
Age 6 years and under (pertussis is required) DTP, DtaP or any combination of DTP or DtaP with DT (tetanus and diphtheria)

Age 7 years and older (Pertussis is not required) Td, DT, or DTP, DtaP or any combination of these

7^th grade Td booster

5 doses at any age, but...4 doses meet requirements for ages 4-6 if at least one was on or after the 4^th birthday
4 doses at any age, but...3 doses meet requirement for ages 7-17 years if at least one was on or after the 2^nd birthday. If last dose was given before the 2^nd birthday, one more (Td) dose is required.

1 dose not required but recommended if more than 5 years have passed since last DTP, DtaP, DT, or Td dose

Measles, Mumps, Rubella (MMR)
Kindergarten

7^th grade

Grades 1-6 and 8-12

2 doses both on or after 1^st birthday
2 doses both on or after 1^st birthday

1 dose must be on or after the 1^st birthday

Hepatitis B
Kindergarten

7^th grade

3 doses
3 doses

Varicella
Kindergarten

1 dose or health care provider-documented varicella disease or immunity
