Doctor's Note

The Doctor's note MUST contain the following:
  • State your child's condition
  • Any restrictions on activities
  • Any use of medical devices
  • Treatment Received
  • Any follow-up care needed
  • Duration of restriction, or use of devices
  • Date at which child can return to school
The Medical Care Referral form with this information notifies the staff to be aware of your child's current condition, therefore the safety of your child from further injuring his/herself can be promoted. These notes can also be used to excuse your child from Physical Education when necessary.

Supportive Devices
If your child comes to school with a special health condition requiring any form of prosthetic device,i.e., crutches, slings, knee braces, casts, wheelchair etc., it is necessary to have a note from a physician which includes important information to the school stating your child's condition and any restrictions, treatment received, follow-up care, and duration of restrictions or use of devices.

PE Excuses
You can excuse your child from PE for up to 5 days for minor complaints and illnesses.  Remember we encourage participation in PE if at all possible.  It is a very important part of your child's day.  If your child needs to be excluded from PE due to a more serious injury or illness, please bring a doctor's note to the Health Office.

Free of Contagious Disease
Remember to ask your doctor for a note if your child is returning to school after a contagious disease such as pink eye, chicken pox, measles, and unexplained rashes.  If you don't have a doctor's note, go to the Health Office before bringing your child to the classroom.

Seizures
A student with seizures must have a note from the doctor before they can swim.
