PLEASE NOTE:  If your child participates in the Free and Reduced Price Meal Program his/her application for 2011/12 school year will expire at the end of the day on Monday, September 24, 2012. You will be receiving a 2012/13 application in the US mail sometime in late July or early August. If you do not receive an application in the US mail, you can pick one up at the Food & Nutrition Services Office at 1651 16th Street, Santa Monica, CA 90405 or call (310) 450-8338, ext. 70228 to have one mailed to your home. All school sites will have applications available in August. Please complete and return this application by Monday,  September 10, 2012 or sooner. It is imperative that you return this application prior to Monday, September 24, 2012, because it can take up to ten(10) days to process an application.

Only one application per family, please! Make sure to include all required information, including:

• Signature
• LAST FOUR DIGITS of your SSN
  If you do not have a SSN please check the appropriate box
• Your complete mailing address including apartment number, city and zip code
• Monthly income, and
• The names of ALL household members, foster children, etc.

Please submit your 2012/13 application by Monday, September 10, 2012. Please be aware that your application for the 2011/12 school year expires at the end of the day on Monday, September 24, 2012, and it may take at least ten (10) business days to process your 2012/13 application. If your child is in Kindergarten and you would like him/her to enjoy a nutritious meal on the first day of school, it is important that you submit an Application for Free or Reduced Price Meals PRIOR to the first day of school.

Applications must be for the current school year (2012/13). Applications from previous years will not be processed.
Please note: It may take up to 10 (ten) business days to process an application..

Parents are encouraged to return the applications as soon as possible so that your child's status will be current in the computer the first day of school. Applications are available throughout the school year and may be submitted at any time.
