Pre-Enrollment Application

Welcome to SMMUSD Child Development Services. To begin the application process for preschool or before/after school care for your child (grades K-3 or 6-7), you must submit an application in order to be eligible for enrollment consideration.

APPLICATION:
Either print the application English, print the application Spanish, fill in by hand, and mail to the address below
OR
Save the application English, Save the application Spanish to your desktop, type in the responses needed, print and mail.

Applications may also be hand delivered at the enrollment office located across the street from the above address at the corner of 4th and Ashland (green building – enter through the driveway).

Once we have received your application, you will be contacted and asked to come into the office to complete your enrollment paperwork. At this time, eligibility for financial assistance will be calculated.
