Instructions for Requesting a Certificate of Insurance

  1. Complete the Request for Certificate of Insurance form.

  2. Obtain a copy of the contract or permit as we must have the stated insurance requirements.

  3. Be sure to include on the request form the name of the person to whom the certificate should be sent, their address, the date of the event, and a description of the event.

  4. Fax the request and a copy of the contract or permit to Risk Management at 310-452-3468. The Risk Manager will review the request and forward it to ASCIP for processing.
